2007 Annual Reports

At the end of each year, pastors are required to complete forms which tell about the church's ministry during the past year and their own ministry. The forms for 2007 were mailed in mid-January 2008. They include the following:

• Minister's report form (a PDF and the fillable Word form)
• Unassigned minister's report form (for licensed UB ministers not currently employed as a pastor in the UB church)
• Local Church report form (a PDF and the fillable Word form)
• Listing of licensed ministers in your church (a PDF and the fillable Word form)
• Women's Ministry Survey (a PDF and the fillable Word form)
• Youth Ministry Survey (the fillable Word form)

Ministers can download them, fill them out, and return them to the Bishop's Office as email attachments (send to: ). Or, you can fill them out on your computer, print them, and return them by regular mail. In either case, you avoid having to complete the forms long-hand.

The deadline: return the forms to the bishop by February 15, 2008.

To ensure that the reports download property, it was necessary to place them together in a Zip file (which most computers can uncompress). The file below contains all six reports listed above, most in both PDF and MS Word format.
